Three reasons your center needs medical physics — and why contracting it usually wins.
Considering bringing in a medical physics specialist? The cost of an external contractor can look nerve-wracking on a budget line. It is almost always worth it — and often cheaper than the alternative once you count everything the alternative actually costs.
Medical physics improves several things at once: quality of care, cost-effectiveness, safety, workflow, and efficiency. For a medical business, those are not soft benefits.
1. Safety
A medical physicist optimizes how your equipment is used. Working with X-rays, electromagnetic fields, laser light and radionuclides carries risk to both patients and staff that an ordinary business simply does not have. Too much or too little exposure changes the care your facility delivers. Physics oversight is what keeps that envelope tight — and what keeps a regulator's visit uneventful.
2. Cost
Contracting is more cost-effective than employing once you account for training and equipment costs — a physicist needs instrumentation, calibration, and continuing education, and those follow the person, not the post. Beyond that, expert oversight lets you schedule equipment downtime deliberately instead of discovering it. That saves money by avoiding patient bottlenecks and by extending the working life of the devices in your care.
The sharper version of this argument: with a median board-certified salary near $245,000 (AAPM 2024) and a fully loaded seat running past $300,000, a full-time physicist is a serious annual commitment. A single-linac center, a rural program, or a department covering a leave rarely has that much physics work to give one person. Paying for the hours you actually consume is not a compromise — for most centers it is simply the correct size.
3. Efficiency
An experienced physicist gives your in-house staff the practices that save time and money long-term — how the equipment should be used, how it should be cared for, when downtime should occur so it does not turn into cancelled appointments and frustrated patients.
And there is the part nobody enjoys but everybody needs: physics expertise keeps a facility compliant with regulations that change more often than anyone would like.
The honest caveat
Contracted physics is not automatically better than employed physics. A large center with enough volume to keep a physicist genuinely busy should employ one. The argument here is narrower, and it is about fit: most centers are not that center, and they have been offered only the expensive option.
Describe the setup — machines, volume, current staffing — and we will tell you honestly what the physics load looks like, including if the answer is that you should hire someone outright.
